We help improve their living conditions, medical care, and wellness activities, reducing the burden of chronic conditions like asthma, COPD, diabetes, and hypertension, and high-risk pregnancies, which disproportionately affect Black and Latinx communities.

Our Community Health Workers know the communities and cultures we serve and are trusted sources. We aren’t just a phone call – we meet our clients in their homes in order to help the whole person and family.
