Isaiah’s Story
How Jordin, AIRnyc Community Health Worker, helped Isaiah
Isaiah (60) when I met him:
Prediabetic with hypertension
Limited movement in his legs and mostly homebound
A lot of pain, especially at night
Interventions I made with Isaiah:
12 Deliveries of Caribbean meal option
Coached on strengthening and light walking
Educated on DASH Diet and at-home cooking lessons
Taught self-monitoring blood sugar and blood pressure
Taught analyzing food labels and portion control
Isaiah now (after only 3 months!):
Regained all the movement in his legs
His doctor moved him off his blood pressure meds with
NORMAL BLOOD PRESSURE!!!
Prediabetes levels are greatly improved
Got a job he loves because he is constantly moving his body
Olivia’s Story
How we helped Olivia (9) and her mom
In the last three months:
Olivia’s apartment had many leaking pipes and a pest control problem
Her apartment’s utilities were at risk
Olivia’s mom struggled to afford transportation to get to her medical
appointments
“In my first month working with Olivia and her mom, I was able to connect them with a food pantry, a utilities relief program, and a metro card aid program so she’d be able to get to the next doctor’s appointment. This was in addition to setting up regular check-ups and bringing in a pest management company for rodents, which could be adding to asthma triggers in their apartment. I’m not done improving this family’s situation, but I feel good about what I’ve been able to do so far.”

Laiza’s Story
Laiza’s pregnancy journey
Laiza and her husband lost their housing while waiting for their twins to be born.
AIRnyc provided home-visits even after Laiza and her husband had moved to an emergency shelter.
AIRnyc communicated with Laiza’s doctor and also applied for stable housing.
Twins Marcus and Viola were healthy upon their birth.
The family moved to safe, permanent housing.
Laiza gave back to other moms by participating in the “Patient Advocacy Group” led by AIRnyc.

Franklin’s Story
How Jennifer, AIRnyc Community Health Worker, helped Franklin
Franklin when I met him:
Lives with his 16-year-old daughter, Jade
Type 2 Diabetes, Hypertension and Depression
He would often not show up to medical appointments
His rent was more than 60% of his income
There was little food and no fruit or vegetables in his apartment
Interventions I made with Franklin:
Applied for affordable housing
12 Home-delivered meals (Hispanic option)
Educated on DASH Diet, guidance on food labels, portion control and healthy foods
Franklin now:
Blood glucose levels greatly reduced
Shows up for medical appointments
Uses his static bike twice weekly
Eats fruits and veggies daily
Smiles more, is more optimistic in general
