Health Advocacy Partners Program
In collaboration with the Department of Health and Mental Hygiene, Health People and New York Public Housing Authority
As part of the HAP program, AIRnyc’s focus is to connect with NYCHA Butler House residents to provide care coordination at both the individual and family levels. Throughout our program, we facilitate educational workshops, offer one-on-one health coaching, connect members with health and wellness activities, and provide referrals for social needs as required.
Community Outreach and Engagement
Our team of CHWs develops outreach and engagement plans using both in-person and virtual platforms to reach our community and promote HAPP activities to NYCHA residents.
We are dedicated to promoting activities and workshops for community members through street canvassing, flyering, postering, emails, phone calls, and social media to reach priority populations.
We also create our own outreach materials based on events or workshops tailored for the community.
Educational Workshops and Health Coaching
Through our HAP program, AIRnyc provides educational workshops and health coaching to NYCHA Butler House residents. In these sessions, members learn more about health and chronic conditions. The topics that we provide education for include but are not limited to the following:
Hypertension
Stress Management
Peer Support
Respiratory Virus: COVID-19, Flu, and RSV
Self-Care
Diabetes
